1. Download and complete the work permit application (link below). Fill it out completely including parent / guardian signature. For "place of birth", please list City, State and name of the hospital.
2. Bring the completed application along with a copy of your birth certificate, baptismal certificate, passport or driver's license to Mrs. Seip ( mary.seip@svpanthers.org) in the Attendance Office.
3. You must appear in person to sign your work permit. 

The office is available for working papers from 7:15 am to 3:00 pm. Please check for holiday or summer hours for any changes.

Work permits are a wallet-sized blue piece of paper. This must stay with the student until they reach the age of 18. DO NOT give your employer the original work permit; only a copy.
