A Reminder to All Parents from the Nurses:
Although it is the end of the school year it is already time to start thinking about preparing for next year. Please open and print the attached form if your child will be needing medication during the school year. All medications, even over the counter such as ibuprofen requires a doctor’s signature as well as a parent signature as indicated on the form. This is our school policy and the law. Completing the form for a med such as ibuprofen or acetaminophen before the start of the school year assures that the nurses can give your child the med when a headache or cramps arise.

Also, state law requires all students entering Kindergarten (or 1st), 6th and 11th grades to have a completed physical examination on record. The attached form must be used. Physicals done over the summer can be faxed directly to the nurses. Please be sure to keep a copy for yourself.

Finally, school law requires all Kindergarten, 3rd and 7th graders to have a dental report on file. Please use the attached form and fax or mail to the nurses over the summer. Extra forms are available in the office over the summer.
